Furthermore, it involves health and social care professionals at various levels from home care nurses and nursing home staff to paramedics, emergency medical services, ED nurses and physicians in primary and tertiary care units. Unfortunately, service chains are often considered complex both by professional and older patients and their families. Maintaining and improving health and function, however, would require services that are reachable, meaningful and effective. In particular, prompt and high-quality care is important in acute care of the aged in order to advance their recovery and to ensure continued living at home. On the other hand, good care of chronic diseases is an important way to maintain physical function and to prevent acute hospital admissions and injuries.<\/span><\/span><\/p>\n<p class=\"\"><span style=\"font-family: Open Sans; font-size: 14px;\">Successful management of older people\u2019s health conditions should base on comprehensive assessment of their situation. This is important as common geriatric syndromes, like memory disorders and frailty, often remain unnoticed in routine care. This far, the benefits of care based on comprehensive geriatric assessment (CGA) have been demonstrated among acutely ill older people and hip fracture patients. Unfortunately, such care is currently rarely available and there are difficulties in recognizing the patients who should be admitted to geriatric units.<\/span><\/p>\n<p class=\"\"><span style=\"font-family: Open Sans; font-size: 14px;\">The purpose of this research project is to assess function, effectiveness and quality of geriatric services both from the viewpoints of the patient and healthcare system. In addition, the project provides knowledge on the care of acute and chronic diseases, particularly among home care clients. The main reseach topics include detection of factors predicting different health outcomes and testing and developing tools for healthcare professionals for assessing older patients and monitoring quality of their care. In wide scale, the project ranges from preventive services to outpatient geriatric assessment, home care, emergency and hospital care, and long-term care. However, at the moment the main focus is on acute care of the aged, including orthogeriatrics, and \u2013 on the other hand \u2013 on the management home care clients\u2019 chronic conditions. Varsinkin terveydentilan tai toimintakyvyn muuttuessa \u00e4kisti hoidon tulisi j\u00e4rjesty\u00e4 joutuisasti ja laadukkaasti, jotta toipumiselle ja kotona asumisen jatkumiselle olisi mahdollisimman hyv\u00e4t edellytykset. Toisaalta pitk\u00e4aikaissairaudet tulee hoitaa hyvin niiden aiheuttaman haitan minimoimiseksi ja \u00e4killisten sairauksien ja vammojen ennaltaehk\u00e4isemiseksi.\r\n\r\nKoska i\u00e4kk\u00e4\u00e4t ihmiset ovat hyvin heterogeeninen ihmisjoukko, hoidon suunnittelussa ja hoitoa toteutettaessa olisi pystytt\u00e4v\u00e4 huomioimaan erilaiset yksil\u00f6lliset terveydentilaan ja toimintakykyyn vaikuttavat tekij\u00e4t. Valitettavasti vanhuspotilaiden erityispiirteet ja geriatriset oireyhtym\u00e4t, kuten muistisairaudet ja gerastenia, j\u00e4\u00e4v\u00e4t yleisyydest\u00e4\u00e4n huolimatta usein huomioimatta tavanomaisessa hoidossa. Lis\u00e4ksi erilaiset l\u00e4\u00e4kitykseen liittyv\u00e4t ongelmat ovat yleisi\u00e4. Laaja-alaiseen geriatriseen arviointiin (comprehensive geriatric assessment, CGA) perustuva hoito ja kuntoutus on vaikuttavaa, mutta satunnaistettujen tutkimusten tulokset eiv\u00e4t toteudu k\u00e4yt\u00e4nn\u00f6n kliinisess\u00e4 ty\u00f6ss\u00e4 toivotulla tavalla.\r\n\r\nHankkeen tavoitteena on tutkia pitk\u00e4aikaissairauksien ja akuuttien sairauksien hoitoa sek\u00e4 toisaalta i\u00e4kk\u00e4ille suunnattujen palveluiden toimintaa, vaikuttavuutta ja laatua niin asiakkaan kuin terveydenhuoltoj\u00e4rjestelm\u00e4nkin n\u00e4k\u00f6kulmasta. T\u00e4rkeit\u00e4 tutkimuskohteita ovat hoitotuloksia ennustavat tekij\u00e4t, joita voidaan hy\u00f6dynt\u00e4\u00e4 hoidon ja palveluiden kohdentamisessa, sek\u00e4 suomalaiseen terveydenhuoltoj\u00e4rjestelm\u00e4\u00e4n soveltuvien mittareiden ja arvioinnin ty\u00f6kalujen kehitt\u00e4minen, arviointi ja implementointi. Hanke kohdistuu erityisesti kolmeen haavoittuvaan potilasryhm\u00e4\u00e4n: kotihoidon asiakkaisiin, sairaalahoitoon akuutin sairastumisen tai vamman vuoksi joutuneisiin potilaisiin sek\u00e4 monisairaisiin i\u00e4kk\u00e4isiin potilaisiin, joille suunnitellaan raskaita toimenpide- tai l\u00e4\u00e4kehoitoja (esim. sy\u00f6p\u00e4potilaat). Furthermore, it involves health and social care professionals at various levels from home care nurses and nursing home staff to paramedics, emergency medical services, ED nurses and physicians in primary and tertiary care units. Unfortunately, service chains are often considered complex both by professional and older patients and their families. Maintaining and improving health and function, however, would require services that are reachable, meaningful and effective. In particular, prompt and high-quality care is important in acute care of the aged in order to advance their recovery and to ensure continued living at home. On the other hand, good care of chronic diseases is an important way to maintain physical function and to prevent acute hospital admissions and injuries.<\/span><\/span><\/p>\r\n<p class=\"\"><span style=\"font-family: Open Sans; font-size: 14px;\">Successful management of older people\u2019s health conditions should base on comprehensive assessment of their situation. This is important as common geriatric syndromes, like memory disorders and frailty, often remain unnoticed in routine care. This far, the benefits of care based on comprehensive geriatric assessment (CGA) have been demonstrated among acutely ill older people and hip fracture patients. Unfortunately, such care is currently rarely available and there are difficulties in recognizing the patients who should be admitted to geriatric units.<\/span><\/p>\r\n<p class=\"\"><span style=\"font-family: Open Sans; font-size: 14px;\">The purpose of this research project is to assess function, effectiveness and quality of geriatric services both from the viewpoints of the patient and healthcare system. In addition, the project provides knowledge on the care of acute and chronic diseases, particularly among home care clients. The main reseach topics include detection of factors predicting different health outcomes and testing and developing tools for healthcare professionals for assessing older patients and monitoring quality of their care. In wide scale, the project ranges from preventive services to outpatient geriatric assessment, home care, emergency and hospital care, and long-term care. However, at the moment the main focus is on acute care of the aged, including orthogeriatrics, and \u2013 on the other hand \u2013 on the management home care clients\u2019 chronic conditions.
